  • The date you started your business
  • Social Security Number
  • EIN Number
  • Your annual revenue
  • Your business mailing address with 4-digit zip code extension
  • Must Initial question 5 & 6 on PPP application
  • Must fully complete page 2
  • Detailed information to calculate the average monthly payroll costs for your employees for the past 12 months, as outlined by the SBA
  • Verification of the number of employees past 12 months
  • Most recent four quarters of Payroll Tax - Form 941
  • State income, payroll, and unemployment insurance filings
  • Documentation verifying payroll costs, mortgage interest, rent, and utility payments you expect to incur over the two months after you receive funding.
  • 2019 Profit & Loss Statement or 2019 Business Tax Returns (if completed)
